The Clinical Pharmacy Specialist (Informatics) performs with full professional authority, knowledge, and judgement with advanced lev! el of responsibility and experience in all aspects of the total Pharmacy operation. The Incumbent is responsible for oversight of all specialized Pharmacy ADP applications.Oversight includes, but is not limited to , programs for clinical Outpatient and Inpatient medication reviews, CPRS, BCMA, medication quality management, medication error reports, and statistical data support. The incumbent serves as the pharmaceutical expert in areas such as medicine, surgery, and geriatrics including recommending, calculating, and preparation of the TPN and PPN nutrition support products, chart reviews, and monitoring/assessing the outcome of drug therapies. He/She may perform 1V admixtures, unit-dose and automated drug distribution systems, procurement, consultation, stock control, research, bulk compounding, prepackaging and storage.
The incumbent is assigned special reports, projects and problem matters, requiring an overall or specialized knowledge of the trends,problems, a! nd other matters affecting the total Pharmacy operation. The C! linical Pharmacy Specialist provides clinical information on a timely basis to all hospital staff and patients. Guides,directs, and supervises Pharmacy technicians. Inspects wards and clinics for outdated, deteriorated or excessive stocks. Handles problems in a discreet manner and ensures a satisfactory solution. Refers problems above his/her authority to supervisor. Assumes pharmacist-in-charge assignments and performs the duties of a supervisory position in addition to regular assignment. Performs special assignments when assigned, such as drug utilization evaluations, and systematic internal reviews.
Salary will be determined by the Professional Standards Board and is based on education and experience.

Qualifications & Evaluations
Basic Requirements:
1. Citizenship. Must be a Citizen of the United States . Non-Citizens may be appointed when it is not! possible to recruit qualified citizens in accordance with VA Policy. 2. Education.
(A) Graduate of a degree program in pharmacy from an approved accredited college or university. The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP). Verification of approved degree programs may be obtained from the American Council on Pharmaceutical Education, 311 West Superior Street, Suite 512, Chicago, Illinois 60610-3537 Phone: (312) 664-3575), or through their Web site at: http://www.acpe-accredit.org/ .
(B) Graduates of foreign pharmacy degree programs or those who graduated from a U.S.-based non-ACPE accredited degree program meet the educational requirement if their degree is found to be equivalent to degree programs recognized by the ACPE. This finding may be based on any of the following:
(1) A letter of acceptance into a United States graduate pharmacy program recognize! d by the ACPE.
(2) Written certification from the Foreign Pharmacy Graduate Examination Commission, 700 Busse Highway, Park Ridge, IL 60068, Phone (847) 698-6227, that the individual has successfully passed the Foreign Pharmacy Graduate Examination.
(3) A letter from a United States college or university with a pharmacy degree program recognized by ACPE stating that the individual's pharmacy degree has been evaluated and found to be equivalent to its Bachelor of Pharmacy degree or higher.
3. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.
4. Must meet Physical Requirements.
5. English Language Proficiency . Must be proficient in spoken and written English.
Grade Requir! ements:
Creditable Experience
(1) Knowledge of Professional! Pharmacy Practices. To becreditable, the experience must have demonstrated the use of knowledge, skills,and abilities associated with professional pharmacy practice. Professionalpractice means paid/non-paid employment as a professional or unlicensedgraduate pharmacist as defined by the appropriate licensing board.
(2) Residency and Fellowship Training. Residency andfellowship training programs in a specialized area of clinical pharmacypractice may be substituted for creditable experience on a year-for-year basis.The pharmacy residency program must be accredited by the American Society ofHealth-System Pharmacists (ASHP). A fellowship program that is not accreditedby the American College of Clinical Pharmacy (ACCP) will need to havecomparable standards for experience to be creditable (Professional StandardsBoard refers to the Deputy Chief Consultant for Professional Practice for thedetermination).
(3) Quality of Experience. Qualifying experience mustbe at a level com! parable to pharmacy experience at the next lower level.Experience as a Graduate Pharmacist is creditable provided the candidate wasused as a professional pharmacist (under supervision) and subsequently passedthe appropriate licensure examination.
(4) Part-time Experience. Part-time experience as aprofessional pharmacist is credited according to its relationship to thefull-time workweek. For example, a pharmacist employed 20 hours a week, or on a1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeksof service.
Grade Determinations: GS-12
1. One year of experience equivalent to the next lower grade level

2. Completion of an ACPE-accredited Pharm. D. program
A. Assignment: A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpre! ting, and verifying medication orders for appropriateness; processing a! nd filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information;assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): 1. Knowledge of professional pharmacy practice.
2. Ability to communicate orally and in writing to both patients and health care staff.
3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
