Five years previous experience in hospital care setting preferred. Prefer oncology certification (OCN or AOCN).
Registration and current licensure as an RN in Mississippi. Job Summary: Offers individuali! zed support to patients with suspicious and positive breast fi! ndings, cancer survivors and their families throughout their diagnosis and treatment process. Must be very compassionate as well as organized and resourceful. The breast health / oncology survivorship navigator must have excellent communication skills and be able to function within a multidisciplinary team approach.
Examples of Essential Job Functions (but not limited to): Develop and implement a process to disseminate a comprehensive care summary and followâ€"up plan to patients with cancer who are completing cancer treatment.Educate patients on prevention of recurrent and new cancers and other late effects.Develop Breast navigator program/processes, utilizing methods such as surveys and cancer mapping.Follow newly diagnosed breast cancer patients from the time of diagnosis until treatment completion.Initiate communication with patients upon learning they have a suspicious mammogram/ultrasound or a positive finding, educating patients with suspicious findin! gs about the importance of follow up examinations.Offer genetic screening, education, and appropriate referrals to applicable patients.Connect patients to community and social support services.
